src/theory/quantifiers and src/theory/rewriterules. This unclutters the src/theory directory somewhat.
The namespaces weren't changed, only the file locations.

(extended) SMT command get-model. added collectModelInfo and removed getValue from theory interface. merge also includes major updates to finite model finding module (from CASC), added fmf options, some updates to strong solver and quantifiers engine interface. The test recursion_breaker_black currently fails for me on production builds, Morgan is planning to look into this.

Adds TheoryQuantifiers and TheoryRewriteRules, QuantifiersEngine, and other infrastructure.
Adds theory instantiators to many theories.

Below is a highlight of the changes:
- This introduces a new normal form to arithmetic.
-- Equalities and disequalities are in solved form.
Roughly speaking this means: (= x (+ y z)) is in normal form.
(See the comments in normal_form.h for what this formally requires.)
-- The normal form for inequality atoms always uses GEQ and GT instead of GEQ and LEQ.
Integer atoms always use GEQ.
- Constraint was added to TheoryArith.
-- A constraint is a triple of (k x v) where:
--- k is the type of the constraint (either LowerBound, UpperBound, Equality or Disequality),
--- x is an ArithVar, and
--- v is a DeltaRational value.

Seems to be working better <http://church.cims.nyu.edu/regress-results/compare_jobs.php?job_id=3749&category=&p=5&reference_id=3739>, and should fix the failing cases in the regressions.
Removing one test case from the integer regress0.

SMT-LIBv1 and SMT-LIBv2 input:
In SMT-LIBv1, you specify the "cvc4_logic" benchmark attribute; for instance:

In SMT-LIBv2, you use a set-info; for instance:

Right now, the only thing this does is disable the symmetry breaker for
benchmarks like the above ones.
As part of this work, TheoryEngine::setLogic() was removed (the logic field there
wasn't actually used anywhere, its need disappeared when
Theory::setUninterpretedSortOwner() was provided).
Also, Theory::d_uninterpretedSortOwner got a name change to
Theory::s_uninterpretedSortOwner, to highlight that it is static to the Theory
class. This represents a breakage of our separation goals for CVC4, since it
means that two SmtEngines cannot be created separately to solve a QF_AX and
QF_UF problem. A bug report is pending.

Dumping infrastructure. Can dump preprocessed queries and clauses. Can
also dump queries (for testing with another solver) to see if any conflicts
are missed, T-propagations are missed, all lemmas are T-valid, etc. For a
full list of options see --dump=help.

Printer improvements, printing of commands in language-defined way, etc.
Typechecker stuff in expr package now autogenerated, no need to manually
edit the expr package when adding a new theory.

The theoryOf is not all in one place, theory::theoryOf. The uninterpreted sorts belong to the builtin theory and are dispatched to the apropriate theory (QF_UF, QF_AX) through theoryOf based on the setting in the Theory class.

* Remove BackedNodeBuilder<> and collapse NodeBuilder<> hierarchy.
Updated documentation in the file. Resolves bug #99.
* Convenience NodeBuilders (PlusNodeBuilder, OrNodeBuilder, etc.)
moved into a separate file. Partially resolves bug #100.
* Moved isAssociative(Kind) into kind.h (and into the CVC4::kind
namespace) instead of metakind.h (where it was in CVC4::metakind).
This clears up a warning (private #inclusion) from the SMT and SMT2
parsers, and maybe makes more sense anyways, since this is based on
the kind (and not the metakind) of an operator.
* Documentation improvement; doxygen top-level \file gestures, \brief
gestures for files, etc. Changed contrib/update-copyright.pl for
this change, and post-processed to add \brief. Resolves bug #98.
* Removed ExprManager::mkExpr(Kind) and NodeManager::mkNode(Kind).
They no longer made sense. Resolves bug #91.

instead of assuming it's atomic based on kind. Atomicity is
determined at node building time. Fixes bug #81. If this is
determined to make node building too slow, we can allocate another
attribute "AtomicHasBeenComputed" to lazily compute atomicity.
* TheoryImpl<> has gone away. Theory implementations now derive from
Theory directly and share a single RegisteredAttr attribute for term
registration (which shouldn't overlap: every term is "owned" by
exactly one Theory). Fixes bug #79.
* Additional atomicity tests in ExprBlack unit test.
* More appropriate whitebox testing for attribute ID assignment
(AttributeWhite unit test).
* Better (and more correct) assertion checking in NodeBuilderBlack.
* run-regression script now checks exit status against what's provided
in "% EXIT: " gesture in .cvc input files, and stderr against
"% EXPECT-ERROR: ". These can be used to support intended failures.
Fixes bug #84. Also add "% EXIT: " gestures to all .cvc regressions
in repository.
* Solved some "control reaches end of non-void function" warnings in
src/parser/bounded_token_buffer.cpp by replacing
"AlwaysAssert(false)" with "Unreachable()" (which is known
statically to never return normally).
* Regression tests now use the cvc4 binary under
builds/$(CURRENT_BUILD)/src/main instead of the one in bin/ which
may not be properly installed yet at that point of the build.
(Partially fixes bug #46.)
* -fvisibility=hidden is now included by configure.ac instead of each
Makefile.am, which will make it easier to support platforms
(e.g. cygwin) that do things a different way.
* TheoryUF code formatting. (re: my code review bug #64)
* CDMap<> is leaking memory again, pending a fix for bug #85 in the
context subsystem. (To avoid serious errors, can't free context
objects.)

recognize an instantiation of the join conversion/copy ctor with
ref_count = ref_count_1 as a copy constructor. Problems with
reference counts ensue.

implementation work. Changed from continuation-style theory calls
to having an data member for the output channel. registerTerm() and
preRegisterTerm() work.

AttributeManager knows nothing of specific attributes, it just as
